State of Incorporation: Check the header or opening section of your document - it will state which state's laws govern your company. Usually, it is found on the first page under Article II of the Incorporation document. You'll also see this in the filing stamp that the state agency placed on the document when it was officially recorded.
Date of Incorporation: Look for the filing stamp (either physical or electronic) on your document. The date shown in this stamp is your official incorporation date.
